COMMERCIAL GENERAL LIABILITY

Does your rink require you to have liability insurance to get on the ice? We'll provide that, and more, protecting your group with up to $20,000,000 of liability coverage for you and your participants.

 

DIRECTORS & OFFICERS LIABILITY

Insure your league executive and team reps with up to $20,000,000 of coverage against legal action that may arise as a result of incidents in the league.

 

ACCIDENTAL DEATH & DISMEMBERMENT

If any unfortunate accident or tragedy results in your league, know that your players are backed by industry-leading AD&D insurance coverage.

 

SUPPLEMENTARY SPORT ACCIDENT COVERAGE

Dental, Medical, Physio, Loss of income, and more; it's all covered under Hockey Canada's supplemental sport accident insurance to protect your players in the event of an injury.

 


 

INJURY CLAIM PROCESS

 

Suffered an injury with your ASHN-registered team? Our five-step process will help to ensure that you're following the proper process to notify Hockey Canada of your injury report and open up your claim.

*Please be advised that all injury reports are subject to review and any insurance claims are subject to the terms and conditions of your ASHN Registration, Hockey Canada's insurance policy, and to be paid within the limits allocated within Hockey Canada's supplementary sport accident insurance policy.

 

1) Contact us to let us know an injury has taken place in your league.

 

2) Download the ASHN Injury Report Form (EN) (FR) and complete the form. Be sure to have your physician or dentist sign complete the second page of the report as well, if applicable to your injury.

 

3) If you have primary insurance benefits, please obtain an Explanation of Benefits and include that with your injury report.

 

4) If you have obtained any receipts of expenses incurred from the injury, please include copies with your injury report.

 

5) Mail the complete package to the Hockey Canada address listed at the bottom of the ASHN injury report. If you incur any further expenses or are not sure what might be covered under the policy, please contact Jayne Livingston for additional information.
